Ultrasound-guided Pulsed Radiofrequency Treatment of Brachial Plexopathy due to Fibroma

Abstract

|
44-year-old man visited our pain clinic with pain, sensory deficits, and motor weakness of the left arm that developed after removal of a fibromatous tumor around the brachial plexus. Thirteen years ago, he had debulking surgery of a huge fibroma around the scalene and trapezius muscles. Following this procedure, he experienced persistent pain of the left arm. Follow-up MRI performed 10 years after the operation showed multiple tumors recurred surrounding the left C5-C8 nerve roots and left upper pleural space. The patient complained of left upper extremity pain, hypoesthesia at the left C5-T1 dermatomes, motor weakness at the left C7-T1 nerves, and Horner¡¯s syndrome. Several times of brachial plexus blocks were given to him at the nerve root level, but the improvement of symptoms was temporary. Therefore, ultrasound-guided pulsed radiofrequency treatment on the left C5-C7 nerve root was performed. There were no noted complications and significant symptom relief was achieved for more than 6 months.
